Our Services

What we evaluate & provide

Each service is tailored to the unique and evolving needs of stroke survivors and their caregivers in the critical period following discharge.

Caregiver Burden Screen
Focuses on including the caregiver as an integral part of health care provided to the stroke survivor. Identifies the needs and wants of the caregiver and includes them in the patient's treatment plan.
Depression Screen
Depression can arise in stroke victims at any time. Our screen not only identifies the presence of depression but assists the caregiver and stroke victim in understanding and identifying symptoms before they become severe.
Emotional Lability Scale
Given to clients who laugh or cry uncontrollably to identify specific conditions that require specific therapeutic approaches.
Medication Reconciliation
Critical to ensure the stroke victim is taking the proper medications and that they are effective. Confusion about medications frequently causes readmission to the emergency room and even emergent situations.
Vitals Screen
Screening individual vitals can help identify problems before they arise and assess the effectiveness of medications currently being taken.
PT, OT & Speech Therapy Recommendations
We provide information directing you to the best neurological rehabilitation outpatient centers in the region and follow your progress via video conferencing.
Equipment Evaluation
When patients are discharged with equipment such as wheelchairs, walkers or canes, our clinic evaluates the safety of that equipment related to the patient's specific disability.
Rehabilitation Needs Evaluation
We confirm or modify the patient's rehabilitation needs based on their evolving recovery over the full year following their stroke.
Gait Evaluation
We provide a brief gait evaluation using the client's equipment to assess and evaluate safety during ambulation and mobility.
Social Work & Support Group Information
Take advantage of our resources to connect the disabled population with social services and support resources in the Maryland region.

Why spasticity can
appear months later

Many stroke survivors are caught off guard by spasticity — the stiffening of arms and legs that can develop up to 9 months after their initial stroke. It is often not discussed during early discharge, leaving patients without the care they need.

Did you know?
Spasticity can arise up to 9 months after a stroke — our clinic monitors for this and provides Botox® treatment to relieve symptoms when it appears.
